Passport Application Procedures

Adult Applicant (16 and older)

• Original birth certificate if born in the U.S. or original naturalization certificate if born outside the U.S., if U.S. passport was never issued, or was issued as a minor
• Previous U.S. passport, if one was ever issued
• Photo I.D. (U.S. Driver’s License or U.S. Identification Card)  If the applicant presents out-of-state primary identification s/he must present an additional ID document.
• One passport photo (Photo service is available at this location for a fee of $12.00)
• Applicant MUST be present at the processing of the application

• Original birth certificate (Long form indicating parents’ names)
• Previous U.S. passport, if one was ever issued
• Parents’ U.S. Driver’s Licenses or U.S. Identification Cards
• One passport photo (Photo service is available at this location for a fee of $12.00 for children and adults age 5 and older)
• Child MUST be present at the processing of the application
• Both parents should be present. In the event one parent is unable to be present, the other parent MUST complete and notarize a “Statement of Consent Form” accompanied by a copy of the back and front of their U.S. Driver’s License or U.S. Identification Card
