Required documents include the following:
- Proof of Miami-Dade County residency
- two of any of the following: driver license, properly executed lease agreement, bank or credit card statement, utility bill (must be dated within the last 60 days)
- EMT certification or higher
- High school diploma or higher degree
- Proof of Age - birth certificate, driver license, passport
- Valid driver license (Class E or higher)
- Non-Smoker's Affidavit (Must be completed and notarized)
